Dear List members: Advice wanted: 26 y/o G2P1001 at 9 weeks gestation with a prior history of cold knife conization and prophylactic cerclage performed during her last pregnancy at 14 weeks because, per the patient, "Cerclage after cervical conization was the protocol at this HMO." Patient had the cerclage removed and delivered uneventfully and had to be induced. Patient is now asking me, "Do I have to have a cerclage this pregnancy?" What is your criteria for prophylactic cerclage after conization? Do you measure the cervix transvaginally? What do you advise patients as the risk of incompetent cervix after cold knife conization? The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
