Thanks for your help. I will have to pull the references. This patient wtih the elevated NTD risk and HCG > 7 mom had her first scan with us. The dates fit and the placenta looks normal. There is no evidence of a lost twin. We dont see syndactyly or any signs of triploidy. We are planning to bring her back to offer an amnio for AFP, ACHE, Karyotype and TORCH PCR. I thought of the torch because that can make hyperactive placentas. We dont have time to assess serology and I think she will want the amnio. We will also watch growth carefully and consider fetal assessment later. Interestingly, our AFP lab is run by the local pathology consortium and they do not report trisomy 21 risks if the NTD risk is high. Thanks again Gary

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
