I think in Mother Nature the baby falls and the cord is torn. Like it happens in animals (think of giraffes...). The tear creates a spasm and the bleeding stops soon. There has been a recent article (Lancet ?) in which a mother delivered alone in her hot bath and kept the baby in the water without clamping the cord. It resulted in a severe polycythemia, the placenta and cord were still active.
