31 YO Primigravida, 5 feet 2 inches, 170 pounds at 11 weeks, has a prenatal course marked by first trimester spotting, subsequently had an elevated MSAFP risk for Down syndrome, with a normal amnio/ultrasound. Her BPs have been 100 to 120/70 to 80 throughout the pregnancy. At 36 weeks she had BPs of 130 to 140 over 90, no protein, no excessive weight gain, and was put on bedrest/twice weekly observation. 4 days ago, she had a BP of 140/90, 1+ protein, a reactive NST, a cervix that was 0/25%/floating/post/firm. Lab at that time showed a Hematocrit of 38, normal platelets, and an SGOT of 35 (upper normal is 40). These results were available on Saturday, and she returned for a visit today (Tuesday). Today, her BP was 140/100, repeated 135/80, 1 to 2+ protein, no change in her weight, asymptomatic. Her cervix was unchanged, and her biophysical profile was normal, with an estimated fetal weight of 3996 grams. What is your diagnosis, if any? What would you do? Garry

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
