Whether the cavity was entered or not, this is a uterus with two prior surgical insults; unless they were clearly pedunculated fibroids, it is much safer to schedule a Caesarean section. Even closely approaching the cavity puts the uterus at risk. It is analogous to deciding whether to allow a VBAC without information on the previous scar. The burden lies with the Ob to prove that the uterus can withstand labor. Scalpel, please. PS: Watch out for those adhesions you may encounter upon opening the peritoneum! The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
