I saw a patient yesterday who is 1 year post mastectomy for poorly differentiated intraductal carcinoma of the breast. Estrogen receptor positive. Patient now age 43. Her mother and sister both had breast cancer as well. No family history of ovarian, uterine or colon. Her surgeon suggested that she have a BRCA1 test done and if positive she should have her ovaries removed for fear of developing ovarian cancer. I told her she was at some increased risk of developing ovarian and colon in light of her breast cancer history, but I was totally unfamiliar with any information supporting the above suggestion. She is easy to examine and I felt that pelvics every 6 months and an occassional yearly TVUS should be more that adequate. Can anyone share any more info or direct me to some studies??? The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
