I usew Gortex graft and suture with 3/4 curved needle. Easier to work in sacrum.No need to dissect off bladder.I have a special dildo like stainless instrument that goes in vagina to elevate same without containination to operator,i.e. has a handle.Stay away from central part of sacrum,i.e. veins and have some sterile thumb tacks available if you get into bleeding(never used). Keep the tension off graft, i.e. let it just lay in the posterior peritoneum opening. First step as you open identify right ureter.Retract lat.Hope this helps.Joe
