Andy, I agree with John. TeLinde's has a nice section on abdominal sacral colpopexy. I usually do a Burch/paravaginal repair when doing an abdominal sacral colpopexy, because changing the vaginal axis with the colpopexy may lead to stress urinary incontinence. I also do a culdeplasty to prevent future enterocele. I have had excellent results with this surgery. My partner's father is also a "mature" ob/gyn and has done well over 400 of these without significant complications.
