Deborah-- My thoughts re your cases: #1) If B/P is normal now, give no meds. Get baseline labs. Watch B/P closely, as superimposed pre-eclampsia is very bad. Meds will only prevent her from stroking out--if you are concerned about this, CONSULT and COLLABORATE. Of course, for HTN/PIH, collaborative management is my routine. Get early (1st trimester) 1 hr glucola. If normal repeat again at 20 and 28 weeks. If abnormal get 3 hr GTT and manage as usual. A dietary consult wouldn't hurt in any case!. #2) You didn't say much about the history given or how reliable you feel the patient is. If the history is vague and the PE is negative, fern is negative, temp is normal, SROM is doubtful. However, due to the AFI of 9.0, I would repeat the U/s. If her history is good or she continues to c/o leaking, I would speak to your consultant. Perhaps he/she would consider injection of dye to r/o leaking of AF. Just my thoughts. Lenora McCall, CNM

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
