To those who consider speed a mark of quality for vaginal hysterectomy, consider the following observations: I do roughly 90 percent of my hysts vaginally. In an easy case (without fixation or enlargement), the uterus is out in 10-15 minutes and the adnexa are both out in another 5-10 minutes. It takes me NO LESS than 20 minutes to create lasting support to the vaginal apex with culdoplasty and lateral suspension suturing IN THE ABSENCE OF PELVIC FLOOR DEFECTS. With prolapse, a good pelvic floor repair takes at least an additional 1-2 hours. I conclude with a high degree of certainty, therefore, that any vag hyst done in 15 minutes or less does not remove the adnexa, does not provide adequate support to the vaginal cuff and does not address pelvic floor defects.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
