I uspect the $171.60 is what th insurance companies in your area have established as the fee for a Level 5 consultation visit. Docs don't set their fees any more, the insurance companies do. Usually the fee that one can get paid for doing an amniocentesis is in the range of $140-250. I would guess that the $1800 includes the Laboratory fee. If all they are doing is a karyotype, that seems alittle excessive at least for the fees that I am aware of in Texas and also around here.
