I would be hesitant to use oral MTX, as the absorption is somewhat unpredictable and saturable. Both poor compliance with IV therapy and use of oral therapy (which is much less reliable) would be prone to causing MTX resistance. Which regimen for MTX is she getting? Obviously high-dose MTX with folinic acid rescue would not be ideal since she is noncompliant and may develop fatal toxicity if she fails to take the folinic acid properly. Would she be open to weekly MTX (50 mg/sq. meter) IM or dactinomycin (1.25 mg/sq. meter) IV bolus q 2 weeks?
