There is no evidence that vaginal birth worsens the prognosis in gravid women with cervical cancer. Cesarean delivery is done to shorten the time to treatment and to prevent bleeding in cases of large tumors. The edema of pregnancy makes the dissection planes nice to work with. Cesarean delivery prior to labor is better because it is much more difficult to define the vaginal margins for a type 1 (extrafascial) hyst or a Meigs-Wertheim resection after significant cervical softening and dilatation has occurred.
