In anyone without her track record, I would definitely go with the methotrexate at 50mg/m2. However, with this being her 4th ectopic I am inclined to recommend strongly to her that we laparoscope and resect the tube and ligate the contralateral one. Explaining the long term plan to her of in vitro fertilization.
