The physicians (and CNMs) in our practice would advise an elective C/S on this patient. In fact we have done so on a few occasions. If she declined and desired a vaginal birth, we would see that she was well counseled on the risks of a recurrent rectovaginal fistula no matter what was done to prevent it.

I don't know about Australia, but in the U.S. the medical-legal issue in this type of case is significant.
