In 14 years of practice and 4 years of residency, I have never seen a case of vaginismus. So, take this for what it is worth. In a 19 year old with pain and a vaginismus response to exam, who denies intercourse but appears to have a non-intact hymen; an initial thought is child sexual abuse or rape; and a psychological or psychiatric denial of its having happened. How can she never have touched her vagina?? Maybe she doesn't know exactly where it is and an anatomy lesson, first with a book and then with a mirror would help. Bernard's suggestion of EUA will give you some information and talking to her mother (if she agrees) might give a clue. Good luck. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
