Ask the general surgeons how much they like doing C/S. Sometimes I think we do these things because no one else wants to. As for the infertility part, it is NOT insured in Canada (well actually investigations such as L/S, PHT, and routine bloodwork are, IVF and most of the "High tech" stuff isn't. As for episiotomies - I have done many forcep deliveries (including mid forcep rotations) without episiotomies, however if I have a bad shoulder dystocia I will probably cut an episiotomy before I intentionally break a clavicle.
