Ashley: A number of companies do provide for this; for example, Aetna US Healthcare does in our neck of the woods. They use a High-Risk CPT code (59403) to pay an increment to the provider who cares for the high-risk patient, added to the global reimbursement (e.g., if the general Ob who delivers the patient provides the care, the enhancement is his/hers; but if the additional care given to the high-risk patient is provided by someone else - perinatologist - the referring provider does not see the increment). The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
