Here is my reply to the episiotomy with shoulder dystocia: I would opt for the ML episiotoy vs. risking a laceration. Leave the scaple away from my perinum if the intention is to do a medial lateral incision. I have observed negative experiences working on the post partum setting taking care of women that have had the later incision by one particular physician that ONLY performs medial lateral. This physician also has the reputation when doing laproscopy, manipulating the instruments as if churning butter! My opinion and remains nulliparous at this time, Tara L.
