My note on midline vs mediolateral etc with shoulder dystocia was garbled to say the least. My comment was that I once had the worst case of SD in which the baby died before the shoulders could be delivered. The epis was not the problem. Once one gets into some of the maneuvers (particularly delivering an arm) whatever needs to give way, will. It seems that the suggestion of doing an episiorectotomy or and large epis will hopefully control the damage one needs to repair. Joanne Bulley

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
