I just induced my first patient (42 wks, long thick high posterior cervix) with cytotec 50 mg. Gave the first intravaginal dose at 6 pm, the second dose at 6:30 AM, at which time cx was now 2-3 cm/50%/posterior. She delivered 6 hrs later a 9#2 boy with apgars 8/9, clear fluid. I planned to give the second dose in 4 hrs, but she contracted every 2-3 min until 5 am or so, so I was concerned about overstimulating her.

My question for those who use cytotec on regular basis: If after the first dose the woman is contracting vigorously (Q 2 min) but no cervical change, do you go ahead with next scheduled dose every 4 hrs?

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
