I've heard Speroff advocate a minimum of 5 days prior to checking a FSH. However, he was referring to OC's used in the perimenopausal patient. I'd be curious whether your patient had any amenorrhea or simply some hot flashed. Too often, I see young patients with hot flashes, and regular menses placed on HRT because their FSH was elevated. Unfortunately, most patients in their 40's will have elevated FSH's and be ovulating on a regular basis. I might consider that if your patient was having regular menses prior to instituting her HRT, merely discontinuation and follow to see if her regular menses resume. Probably a lot more informative than an FSH.
