Yes, I would first try 200 mg of micronized progesterone for 10 or 12 days each month. If she does not tolerate this, going to progestin every three months with annual sampling of endometrium would be a good alternative. I recently saw a 68 year old who had a endometrium with complex hyperplasia with atypia with an endometrial stripe of 4 mm., thus I no longer feel confident that ultrasound for endometrial thickness tells me what I need to know re the status of the endometrium.
