HRT/pellet implantation

I am an ob/gyn office/surgical nurse and have 4 estradiol(25mg each)/1 testosterone (75mg)implanted q 3-4 months. Our office is having a difficult time in coding this procedure so that insurance companies will cover it. I know most of the physicans do not handle this end of the business, but any info would be appreciated.

And for those physicans doing this procedure, how often do you repeat implantation on your patients? Information we first received recommended 4-6 months but we are finding that the pellets are lasting only 3-4 months.
