Robert:

If you look in the archives of this forum, I had a pt with the identical clinical situation and placental histology. She lost *two* at 22-24 weeks.

An MFM consultant opined that a uterine artery spasm or chronic high resistance situation may be contributing. This was documented by 1st trimester uterine artery Doppler flow studies showing absent end diastolic flow. This has resolved on follow up Doppler after initiation of Ca++ channel blocker therapy. Thus far on this, her third pregnancy I've got her to 33+ weeks, doing well by all accounts with weekly NSTs/fundal height measurements/ultrasounds.

She has been on the following regimen from the 6th week:- ASA 81mg po qd Subcutaneous heparin 5000 units tid nifedipine 120mg [long acting] qd

Hope this helps

Hugo

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
