Previous intra-uterine demise @18 -20wks. Had "placenta previa". Bled for 10wks prior to IUFD. Delivered with prostin suppos. Had work up including hysteroscopy which was negative. TORCH titers negative ANA negative. Presents to me with 2nd pregnancy, progesterone levels normal. Starts bleeding at 10 weeks- U/S shows marginal placenta previa. Bleeding slows then stops. Followup ultrasound at 17weeks- appropriately grown fetus, low-lying placenta. Triscreen normal. Routine visit at 19weeks - intra-uterine demise. Delivered with prostin suppos. No gross fetal anomaly. Awaiting fetal chromosome studies. Maternal anticardiolipin studies, Lupus anticoagulant negative. TSH normal. Placental pathology: findings suggest posibility of" maternal floor infarction" a condition of uncertain etiology in which there is increased fibrin deposition of the decidua basalis with some extension of the process into adjacent villi . It is associated with a high fetal mortality and can recur in successive pregnancies". The pathologist could not give more references than those in Bernishke's Placental Pathology. She will seek local MFM consultation. Any ideas on this condition? Candidate for empiric therapy with aspirin + or - heparin? Thanks in advance for your thoughts.
