I've got a 62 year old woman one year out from stage 1 breast cancer rx on tamoxifen for adjuvant rx with postmenopausal bleeding and an abnormal tranvaginal scan. The area of the endometrium shows the typical complex, thickened pattern seen with tamoxifen. The biopsy shows minimal tissue. Having seen these before, the sonohysterogram is likely to demonstrate the changes are outside the endometrium. The sagittal measurement of the endometrium using the conventional landmarks is 30mm. My question: where in the literature can I go to get an idea what to suggest to the oncologist giving the tamoxifen and the patient who is concerned about the postmenopausal bleeding. Should she stop the tamoxifen? Should she have a hysterectomy and stay on it? Thanks for any help you can give.
