Re: BPD and lung maturity?From: Geffrey H. Klein, MD (gklein@bcm.tmc.edu)Fri Jan 31 21:36:23 1997
>My partner is sending a 37-ish week G2P1. prior c/s (choosing elective >repeat) for an US Mon when I'm in the ofice. The repeat c/s is >tentatively scheduled for Thurs. My partner asked me to do an amnio for >lung maturity if the BPD isn't "mature by the USC criteria". >What is she talking about? Is there a BPD that correlates highly with >lung maturity? Did I miss that lecture in med school? I am going to "quote" from the ACOG Educational Bulletin #230 Assessment of Fetal Lung Maturity (Nov 1996), If you want to read the whole thing, you will have to be an ACOG member.. Imaging Ultrasonography has replaced radiographic techniques for assessing fetal age. A biparietal diameter of 9.2 cm or more, a femur length of 7.3 cm or more, the presence and size of epiphyseal ossification centers, placental grading (classification based on chorionic convolutions and calcifications), and other imaging determinations have been evaluated as means to assess maturity (19-21). Some fetuses at term, however mature, may lack these findings, and some fetuses with these characteristics may be immature (eg, maternal diabetes complicated by macrosomia). In general, ultrasound assessment of gestational age (indirectly evaluating for maturity) in the third trimester is inferior to the other methods available. 19. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
