She should receive a loading dose of MgSO4 (4-6 grams) and 2-3 gms/hour on admission. She should then have labs drawn including DIC profile, pre-eclampsia profile, to r/o HELLP syndrome. Dizziness and nausea suggest increased intracranial pressure. Having had a blood pressure that high, she could have an intracranial bleed and should have an MRI or CT scan to r/o a bleed. If scans are negative and labs don't show critical levels (e.g. platelets below 50,000), then give steroids, treat the blood pressure and try to get 24-48 hours to give the steroids time to work for lung maturity.
