Where midwives and obstetricians work well together, both understanding the importance of their own role and being open to the clinical competence of the other then the women get really good and effective care. When I went into independent practic 10 years ago it was easier to keep the women at home for longer when it might have been marginally safer to transfer them to hospital, because the obstetricians were so negative towards home births and women's choice of midwifery rather than obstetric care. Now the climate in England has changed and it is easy to transfer women to hospital and remain caring for them. So women transfer very happily on our suggestion. Similarly obstetricians, who used to find it hard to believe that a woman had laboured exactly as we said and had to make sure themselves, now believe us when we say we need their help and the woman needs augmentation or perhaps operative delivery. They are beginning to understand the midwife notes, which are more full of detail than the cryptic obstetric notes. It is important for us all to learn from each other, then we won't have the horror stories to tell on each other: which we could all tell from times gone by. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
