Regarding urge to push: If occuring in woman who is 6 cm or less, I get them on hands and knees and try to help them breath through the push feelings. If she has the uncontrollable urge to push, I ask her to push just enough to take the pain away, not give it all she's got. If this doesn't work, and the cervix is swelling, sometimes need intrathecal to stop the pushing and allow cerviical swelling to abate. If woman is multip and > 6-7 cm, and gotta push, she usually just dilates and births. (I get ready, no matter how her cervix feels!! I don't see cervical lacs in these situations. Only time I've seen cervical lacs is when operative maneuvers are needed such as forceps, or (rarely) from reducing anterior lip. (this is my first try at writing from the OB-Gyn.net page. Hope it works!)

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
