Regarding urge to push: If occuring in woman who is 6 cm or less, I get them on hands and knees and try to help them breath through the push feelings. If she has the uncontrollable urge to push, I ask her to push just enough to take the pain away, not give it all she's got. If this doesn't work, and the cervix is swelling, sometimes need intrathecal to stop the pushing and allow cerviical swelling to abate. If woman is multip and > 6-7 cm, and gotta push, she usually just dilates and births. (I get ready, no matter how her cervix feels!! I don't see cervical lacs in these situations. Only time I've seen cervical lacs is when operative maneuvers are needed such as forceps, or (rarely) from reducing anterior lip.
