I would have agreed wholeheartedly until the last 3 months when I've had 2 >50 yo women with hysters for benign indications and no h/o abnormal paps,
both of whom had loSIL on vaginal cuff pap and HPV and/or mild dysplasia on colposcopically directed biopsy. Interestingly, one of these women was carrying on an affair with a married neighbor, and the other had a new sexual partner after several years of widowhood. maybe skipping the vaginal pap is okay if the woman is in a long term mutually monogamous relationship, but if not, maybe we should still do them (how about every 2-3 yars?)
