How about some input on this ER hit from last night: 23 yo G2P2 w/ 1 month h/o irregular heavy bleeding and 1 yr h/o vague RLQ pain, presents to ER w/ acute exacerbation of abd pain, worse now in the RUQ. Mild assoc n/v, 1 episode of diarrhea. H/o chole in past. H/o "borderline" HTN and diabetes. Has been checked in the clinic (it's after 5 so nobody's there) Exam: morbidly obese, moderate discomfort post analgesics no palpable masses abdominally or on pelvic (BUT, exam is definitely limited by obesity -- this is a BIG girl) transabdominal ultrasound ordered by ER doc shows several cystic structures. Rads says there is a 8cm cyst inferior to margin of liver and 2 4cm cysts in the lower abd/pelvis but he "doesn't think it's ovarian in nature". CT scan (no contrast d/t h/o anaphalactic rxn to seafood) confirms the above. Again the radiologist says he doesn't "think" these masses arise from the ovary. I look at the films and I'm not so sure that they don't, but i'm not a radiologist and these films are hard to read 'cause of the obesity (don't ask me why a transvag scan wasn't done as well). Any thoughts??

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
