Twins with one IUD

Another bummer! 23 yr old G2P1 - previous NVD. Presented to me by referral from her GP on 07/11/96 with twin pregnancy at 28 weeks gestation. Problems: 1. Severe anemia of unknown origin ( HB 8g% and responded well with Fe++ supplementation) 2. IUD of one of the twins - the alive one seems OK and is growing well acc. to ultrasound (diamniotic, dichorionic). Mother is A+ and otherwise healthy. All blood examinations are normal untill now (except HB).

Questions: 1. What will be the best time to deliver the alive baby? 2. How important is the clotting problems mentioned in all the handbooks - I have seen a lot of IUD's, but never a severe problem in clotting. What is the best way to follow up and diagnose this problem? Platelet count, fibrinogen, FDP's, monomers, full clotting profile - and how often do you have to check it???

Thanks rvdcolf@iwwn.com.na
