You just CAN'T anticipate emergency extraction. A quiet (even not quite absolute) regional anesthesia is better than a general. You always can raise the dose of anesthesic drugs in the catheter to perform a CS. As for emergency extraction without any anesthesia I do agree that it's quite possible, I did two of those in the last six months but how many are we still able to do that?
