I have had a patient who had Paragard put in 8 months ago, did fine until 2 months ago when she started bleeding pretty much on a daily basis. Not pregnant. Responded to progestin and stopped bleeding, then started up again and wouldn't respond to progestin or NSAID. No pain or fever. IUD string has been visible all along. Ultrasound shows part of the IUD in the cervical canal, top of the IUD is about 2 cm from the fundal part of the endometrium. IUD has been removed. My question is, if her bleeding was due to an abnormal position of the IUD, why did she respond to progestin in the first place? I'd appreciate your input. Paul H. Eun, MD, FACOG Puyallup, WA

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
