delivery of the impacted head

the methods to elevate the head everyone has mentioned are certainly appropriate. my major problem has been from lifting the head through the incision and tearing the lower segment. My bias is that this tearing occurs from the hand in the incision, rather than the head itself. Although i have used forceps for delivery of the head in the past, vacuum applied after the head is elevated, along with some patience, seems to allow even less lower segment damage.
