make it a rule to do pelvic exam before any cs and puh up the vertex from below and your problems are over. if it is an emergency, have someone else...anyone else push the head up. i have been able to deliver be cs a head almost on the perineum by doing so. if you have experience in midforceps, give her a block and a large epis and pull the baby out. if impossible, push up the head as above
