We sometimes have a nurse give pressure from below, pushing the vertex cephalad. Other times, it just takes patient, constant, persistant pressure from operators hand, pushing cephalad. The fundus will relax enough to permit the delivery of the vertex. Delivering the breech first doesn't seem like a bad way to go, since a lot of these cases result in lateral cervical tears when the operator gets too anxious and starts jerking the hand that is attempting to elevate the vertex.
