In this patient is hyperplasia a pathologic diagnosis or from visual inspection alone? Often patients who have been treated with progestins have a very thickened hyperemic appearing endometrium. Almost like a decidual reaction in pregnancy. An endometrial biopsy should be done. You may find an intense lymphocytic infiltration in which case hormonal therapy may not be the answer. Is the patient interested in future reproductive function? It is rather unusual to have a premenopausal woman with Sjogren's Syndrome. Does she have a normal uterus with respect to size and configuration? An MRI may demonstrate adenomyosis or small miliary like fibromas. Perhaps an ultrasound would be another diagnostic test to consider. What other manifestations of Sjogrens does she have? Interesting case...let us know more details.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education. OBGYN.net believes these interaction in this forum meets these criteria. For further information see the AMA web site.
