Just to give a different view. I have seen one case where the initial endovaginal sonogram was diagnosed as an early IUP at approximately 4.5 weeks by gestational sac measurement. The patient was subsequently (2 days later) referred to our lab (radiologic) where we usually did abdominal as well endovaginal imaging. We found an eight week abdominal pregnancy that was well above the fundus of the uterus that had been missed by the endovag only exam, there was a small fluid collection in the endometrial cavity. I think that this is the reason that many prefer to do the abdominal scan in these cases. I might add that, in the radiologic practice I worked for, a pelvic sonogram was charged as a pelvic sonogram regardless of whether we used every transducer we owned or not. Our feeling was that we did what ever was required to get the Dx, and it did not matter if we were doing abdominal, endovaginal or both. We also always examined both legs when looking for DVT, but only charged it as one exam. Of course that was in the *old* days of fee-for-service, before managed care hit and I headed for academia.
