Rick and anybody else who's interested:

Dennen's Forceps Deliveries, 3rd edition (there might be a 4th by now) FA Davis Co., Philadelphia, 1989.

There's an entire chapter on Kiellands. Unfortunatly there's not a chapter on dealing with the lawyers.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit.
