TTS and septostomy

Since my fellow investigators have been actively discussing the septostomy protocol for TTS, I thought I should put in my two cents.

The procedure is performed under ultrasound guidance. A 22-gauge needle is used to make a SINGLE hole in the membrane. The membrane should not be fenestrated with multiple punctures as this may result in a large window with the risk of cord entrapment through the defect. A case of this complication from Saudi Arabi was reported at the PUBS meeting last year by Giancarlo Mari after he used an 18-gauge needle to perform the septostomy.
