A response from an ob-gyn friend to whom I forwarded the question. There is also a good article by Nahid Toubia with illustrations, for which I don't have the reference but is easily found through medline. A friend of mine did about two weeks ago and I happened to be on call that night. I wasn't there for the delivery but she showed me the repair. Apparently, she slipped her finger underneath the "hood" that had been created by sewing the upper parts of the labia together and cut along the midline. The urethra sits underneath, so she was just cutting the labia. Then, she only needed a small episiotomy to get the baby out. She didn't sew the labia back but rather oversewed the cut edges, leaving them separate. It's hard to explain without showing you. I definitely think he should wait until delivery before dealing with it and I would advise against cutting near the urethra. As I said, he should just cut the overlying skin and do a generous epis. Epidural anesthesia is ok, but local would work too.

The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
