This is in resposne to the questions and comments about puncturing the dividing membrane in cases with stuck twin syndrome or oligohydramnios/polyhydramnios sequence (We now refer to this procedure as septostomy). Dr. Belfort and I first performed this procedure at Baylor College of Medicine and we presented the first two cases at the Society of Perinatal Obstetricians in 1995. The abstract was published in the Am J Obstet Gynecol 1995;172:429. Amanuscript was submitted to Obstetrics and Gynecology to be considered under the section Instruments and Methods. The editor, however, rejected it and felt it should be submitted as a case report. I have written a chapter for a Fetal Therapy textbook scheduled to be released soon and edited by Ken Moise and Nicholas Fisk. Several individuals have tried this method, including Dr. Lynn Montgomery, with good results. We have a larger series now which was accepted for oral presentation at the 1997 meeting of the Society of Perinatal Obstetricians and the plan is to submit it as a manuscript. When the procedure is succesfull, there is complete resolution of the oligohydramnios/polyhydramnios sequence, resolution of hydrops if present, and catch-up growth. I am now at The University of Texas Medical Branch in Galveston, Texas where we have used the septostomy in one more patient few days ago. George R. The American Medical Association is no longer designating CME hours for AMA Category II CME credit. However, physicians themselves may self designate learning activities as Category II CME credit hours if they feel it is of sufficient educational merit and meets the formal definitions of continuing medical education.
