I do not recall seeing any new FDA guidelines on hysteroscopy pressure. There is a consumer bulletin on hysteroscopy for fibroids (http://www.fda.gov//bbs/topics/CONSUMER/CON00262.html).

There is an older product warning reporting gas embolism in patients undergoing operative hysteroscopy using gas distension which noted that operative procedures should be done with liquid media.

I feel pressure should be kept below 75mm Hg to minimize the chances of overdistention or vascular intravasation. If you need to clear blood or debris, you can either overdilate the cervix, allowing outflow, or use a continuous flow hysteroscope.
