Because the patient is on long term therapy with a progestin, Megace. I suggested continuous, not cyclic, therapy with Megace. I am aware that some have suggested that the “anti-estrogenic” effect of progestins on bone mineralization decreases over time, but I would error on the side of caution and screen for osteoporosis.

If this patient has Stage I breast cancer and is postmenopausal, she has a high probability of long term survival.

I am not aware of any study demonstrating that Tamoxifen is as effective as estrogen at preventing osteoporosis.

I agree with and appreciate your point that Tamoxifen has a positive impact on bone mineralization.
