I was the person who asked if people would place a pessary at 6 weeks in a G2P1 with a previous history of the uterine incarceration. The first incarceration was discovered at 16 weeks when the patient went into urinary retention. It took two weeks of various procedures and literature searches (while the patient straight cathed herself) until the uterus was reduced successfully under epidural anasthesia. Well, as it turned out I am the patient (also a physician). My OB and I decided to go with the pessary at 6 weeks. We used a Hodge and I saw her at least weekly. Initial fittings were not fun, but within a week or two we came up with a size that was tolerable and also seemed to hold the uterus up. Well, by 13 weeks I felt I was developing urinary symptoms again and peeked with the US. I then had it confirmed, and once again, despite wearing a pessary, my uterus was incarcerated. Manual reduction and reduction with a vaginal probe again proved impossible - the uterus did not budge. So, once again I had my uterus successfully reduced under epidural anasthesia. Well, so much for using the pessary! My OB now has experience with 2 incarcerations thanks to me. So much for this being an uncommon condition that is unlikely to recur. So far I am 2 for 2.
