We use Indomethacin as a third-line drug (after beta-mimetics, MgSO4 and/or nifedipine), but do not start it after 28 weeks. Our rationale for this is that Gamissans in Israel reported a 3% rate of irreversible ductal closure with pulmonary hypertension, and we had a case at 30 weeks of our own, where we ended up delivering a sick premie instead of a healthy one. Our survival rate at 28 weeks is 95% in our nursery, which is how we chose our cutoff point. When we use it, we obtain our first echo at 48 hours, repeating them every two days at first, then twice weekly. I don't believe we have continued it after 33 weeks in anyone.
