I would be guided by the patient's symptoms, not her age. Since this is not a surgically induced menopause, I'd start with the lowest dose recognized to be effective for osteoporosis prophylaxis (Premarin 0.625 mg or its equivalent). If she remained symptomatic, I'd up the dosage. If Sx still persisted, I would consider another form of estrogen and/or different route of administration.

However, if the menopause was surgically induced in a 43 y.o., I'd begin with 1.25 mg and attempt to wean the dosage at some point remote from the surgery.
